I grant The Everyday [PIC]tory, LLC the right to take photographs of me, my family and, if applicable, my property in connection with the above-identified subject(s). I authorize The Everyday [PIC]tory, LLC to copyright, use and publish the same in print and/or electronically.

I agree that The Everyday [PIC[tory, LLC may use such photographs of me (and my family) with or without my name and for any lawful purpose, including but not limited to such purposes as publicity, illustration, advertising, and Web content.
